What this scan measures

The thyroid gland in the front of the neck — its size, texture and blood flow — along with any nodules, and the lymph nodes in the neck around it. Each nodule is measured and described by its composition, echogenicity, shape, margin and any internal calcification, which is what determines whether it needs anything further.

Why a doctor orders it

It is ordered when a lump is felt in the neck, when thyroid blood tests are abnormal, and when a nodule turns up incidentally on another scan. Ultrasound cannot tell you whether the thyroid is over- or under-active — that is what TSH, T3 and T4 are for — but it is the best test for structure.

Symptoms that lead to this scan

  • A visible or palpable swelling in the front of the neck
  • A nodule found during a routine examination or another scan
  • Difficulty swallowing, or a hoarse voice
  • Abnormal thyroid function tests
  • Follow-up of a known nodule or after thyroid surgery

Understanding your results

Thyroid nodules are extremely common and the large majority are benign — many people have one without knowing. Reports increasingly use a risk-category system (such as TI-RADS) that combines the ultrasound features into a recommendation: observe, repeat in a defined interval, or sample with a fine-needle aspiration. A description such as 'heterogeneous echotexture with increased vascularity' is a pattern often seen in autoimmune thyroiditis and is read together with your antibody and hormone results. The scan describes appearance; only a biopsy characterises a nodule definitively.

Why the preparation matters

No preparation at all — no fasting, no fluid restriction. Wear something with an open neckline and leave necklaces at home, since the probe needs direct contact from the jaw down to the collarbones. You will be asked to lie with your neck extended and to swallow during the scan, which lifts the gland and lets the lower poles be seen.

Ultrasound uses sound waves, not radiation, and there is no known harmful effect at diagnostic settings — it is the only cross-sectional modality used freely in pregnancy and repeatedly in children. Operators still follow the ALARA principle, using the lowest output for the shortest time needed, particularly in obstetric scanning. There is nothing to recover from and no restriction afterwards. The main limitation is not safety but operator dependence: the quality of an ultrasound depends heavily on the person holding the probe.

Tell the team before your scan if any of these apply
No absolute contraindications
There is no medical reason ultrasound cannot be performed on anyone, at any age, including in pregnancy. The limitations below are about image quality and consent rather than harm.
Open wounds, burns or dressings over the area
The probe needs direct contact with skin through gel, so a recent surgical wound, a burn, an ulcer or a plaster cast over the region may make the scan impossible or require it to be deferred until the dressing can be removed.
Bowel gas and body habitus
Sound does not travel through gas, so gas-filled bowel can obscure the pancreas, aorta and pelvic organs, and depth reduces resolution in larger patients. Fasting and a full bladder are used to improve the window; where the view remains inadequate, a CT or MRI is recommended instead.
Transvaginal or transrectal scanning requires consent
Internal probes give far better resolution of the pelvis, but they are always optional. They are not used without explicit consent, and a transabdominal approach is offered instead — including where an internal scan would be inappropriate.
Not a substitute for mammography in screening
Ultrasound complements mammography rather than replacing it: it cannot reliably show the microcalcifications that mammography detects. For symptomatic assessment in younger women it is the correct first test; for population screening it is not.
Fetal sex determination is illegal in India
Under the PCPNDT Act, no person or centre may disclose the sex of a fetus at any stage, and no request for it will be entertained. This applies to every obstetric scan and does not affect any other part of the examination or its report.
