Understand it first

About refractive error — short sight, long sight and astigmatism

A refractive error means the eye's optics do not focus light onto the retina: an eyeball that is too long or a cornea too steeply curved gives short sight (myopia), too short or too flat gives long sight (hyperopia), and an unevenly curved cornea gives astigmatism. Laser vision correction reshapes the cornea so the focus lands on the retina. It corrects the refractive error you have today; it does not stop the eye ageing, so reading glasses are still likely after about forty, and it does not remove the retinal risks that come with high myopia.

Surgery is not the first step

When is surgery actually needed?

Spectacles and contact lenses are the first and entirely reasonable answer, and most people never need anything else. Laser correction is elective surgery on a healthy eye, so candidacy is strict: a prescription stable for at least twelve months, age generally over eighteen and preferably over twenty-one, adequate corneal thickness, a normal corneal topography with no sign or suspicion of keratoconus, a healthy retina, and dry eye that is controlled first. Pregnancy, breastfeeding, uncontrolled autoimmune disease and unrealistic expectations are all reasons to defer or to decline. A screening that turns you down is the screening doing its job — the complication it is preventing is not reversible.

The technique

Why laser, not open surgery

LASIK creates a thin hinged flap in the cornea — with a femtosecond laser in bladeless LASIK — and lifts it so that an excimer laser can remove a precisely calculated pattern of tissue from the corneal bed. The flap is then laid back and adheres without stitches. Because the surface epithelium is largely preserved, vision recovers fast and there is little discomfort. Surface treatments (PRK, transPRK) and lenticule extraction (SMILE) avoid making a flap at all, which is the better choice for thinner corneas or for people in contact sports.

Before your procedure

Stop soft contact lenses at least one to two weeks beforehand, and longer for rigid or toric lenses, so the cornea returns to its true shape — measurements taken on a distorted cornea produce a wrong treatment. Corneal topography, pachymetry, a dilated retinal examination and tear-film assessment. Blepharitis or dry eye is treated first. No eye make-up or perfume on the day, and arrange a driver.

After you go home

Use the drops exactly as scheduled. Do not rub your eyes at all for a month — this is the single most important instruction after LASIK. Shields at night in the first week. No swimming for two to four weeks and no contact sport for four. Use lubricants liberally. Attend every review, and report pain, sudden blurring or a sharp increase in redness immediately.
