About gynaecomastia (male breast enlargement)
Gynaecomastia is benign enlargement of glandular breast tissue in men, driven by a relative excess of oestrogen effect over androgen effect. It is normal and self-limiting in newborns, at puberty — where it usually settles within six to twenty-four months — and again in older age. Two distinctions matter before anything is treated: pseudogynaecomastia, which is fat alone with no gland and needs weight loss rather than excision; and the rare male breast cancer, which is typically hard, off-centre, fixed, and may come with skin or nipple change.
When is surgery actually needed?
The first step is a cause, not an operation: a proper history, a drug review, examination of the testes, and where indicated liver, kidney, thyroid and hormone tests. Stopping a causative drug or treating an underlying condition can resolve recent-onset gynaecomastia completely. Pubertal gynaecomastia is normally observed, because most of it settles by itself. Weight loss is essential where fat is the main component. Medical therapy has a role in the tender, early proliferative phase in selected patients. Surgery is for gynaecomastia that has persisted beyond roughly a year, has become fibrotic and will not regress, or causes significant distress — and only once the cause has been addressed, because operating without doing so simply invites it back.
Why liposuction, not open surgery
Day-care correction combines two steps because neither alone is sufficient: liposuction removes the fatty component and blends the contour into the chest wall, and direct excision through a small incision at the lower border of the areola removes the firm glandular disc that liposuction cannot. A deliberate cuff of tissue is left under the nipple to avoid a dished, saucer-like deformity — over-resection is as much a bad result as under-resection. The excised tissue goes for histology, and a compression garment afterwards manages fluid collection and helps the skin redrape.

Before your procedure
Hormone and drug review. Stop anabolic steroids well in advance and be honest about them with the surgeon — it changes both the assessment and the recurrence risk. Stop smoking; nicotine directly threatens the blood supply to the nipple. Reach a stable weight. Buy the compression garment, have planning photographs taken, and arrange a driver. Nothing to eat for 6 hours and clear fluids up to 2 hours before.
After you go home
Wear the compression vest exactly as prescribed for the full period — not until it becomes annoying. No chest or upper-body exercise for four to six weeks. Sleep semi-upright early on and keep wounds dry for 48 hours. Massage the scars once healed if advised. Report increasing one-sided swelling, fever or a dusky nipple immediately, and make sure you see the histology result.
