About localised, diet-resistant fat deposits and body contour

Liposuction removes subcutaneous fat from defined areas in order to change contour. It is a body-contouring operation and not a weight-loss treatment: it does not treat obesity, and it cannot touch visceral fat — the fat inside the abdomen that actually drives cardiometabolic risk. What largely determines the result is not how much fat comes out but how well the skin retracts over the new shape, which is why skin quality and elasticity are assessed as carefully as the fat itself.

Surgery is not the first step

When is surgery actually needed?

Diet, resistance training and a stable weight come first without exception. Results are best and safest in people close to a stable target weight with good skin tone, and anyone still actively losing or gaining should wait — the fat that remains will change shape around the result. Liposuction is a reasonable option for disproportionate deposits that have not responded to sustained lifestyle effort. It is not appropriate as a treatment for obesity, for unrealistic expectations or body dysmorphic concerns, or where the real problem is loose skin, for which an excisional operation such as abdominoplasty is the honest answer. Uncontrolled diabetes, significant heart or lung disease, clotting disorders and smoking are all reasons to defer or decline.

The technique

Why vaser, not open surgery

VASER liposuction first infiltrates the area with tumescent fluid containing dilute local anaesthetic and adrenaline, which numbs the tissue and limits bleeding. Ultrasound energy is then used to selectively emulsify fat while largely sparing nerves, blood vessels and the connective-tissue framework, and the emulsified fat is aspirated through small cannulas. The claimed advantages over conventional suction are easier work in fibrous areas and some degree of skin retraction. The volume that can safely be removed in one sitting is limited by fluid shifts and physiology, not by how much you would like taken.

Week by week after liposuction surgery

Day 0

Desk work for most people.

Week 2–6

Most swelling gone by three months; final contour at six. Judging the result earlier is misleading — and weight regain will re-expand the fat cells that remain, so maintenance is part of the treatment.

Before your procedure

A stable weight for at least three to six months. Medical fitness assessment with bloods and an ECG where indicated. Stop smoking and all nicotine. Stop non-essential supplements and blood thinners on medical advice. Planning photographs and area marking while standing. Buy the compression garment in advance. Have the realistic-goals conversation and have it documented. Arrange a driver and somebody at home for 24 hours.

After you go home

Wear compression as prescribed, typically continuously for the first weeks. Walk from day zero and keep well hydrated. Expect leakage for the first nights. No strenuous exercise for four weeks. Manual lymphatic drainage if advised. Report calf pain, breathlessness, fever, spreading redness or dusky skin urgently. Maintain your weight — the result depends on it more than on the surgery.
