What this test measures
Everything in the essential panel plus the parameters that turn a screening test into a risk assessment: free T3 and free T4 with anti-TPO antibodies (the autoimmune thyroid markers), a full iron study with ferritin, and bone chemistry — calcium, phosphorus, alkaline phosphatase and vitamin D — read together.
Clinicians order the advanced version when the basic panel is likely to be insufficient: an existing diagnosis being monitored, a strong family history, symptoms that a 70-parameter screen has already failed to explain, or simply an adult over 40 for whom the extra thyroid antibody and bone markers change what happens next.

Understanding your results
The additions mostly answer 'why', not 'whether'. A raised anti-TPO alongside a high TSH commonly points to an autoimmune cause for an under-active thyroid rather than an iodine or medication one — a distinction that changes monitoring, not a diagnosis you can make from the report. A low ferritin with a normal haemoglobin suggests iron stores are running down before anaemia appears. All of it needs clinical correlation.

Why the preparation matters
Ten to twelve hours of fasting, for the same lipid and glucose reasons as the essential panel. Two extra points matter here: biotin supplements (common in hair and nail formulations) can interfere with several hormone immunoassays including thyroid tests, so mention them, and take thyroid medication AFTER the sample rather than before, so the result reflects your usual state rather than the peak after a dose.
