What this test measures

Eighty-five parameters chosen for what changes after 60: heart and lipid markers, kidney function with an estimated GFR, bone chemistry and vitamin D, thyroid hormones, a complete blood count, and PSA for men.

Why a doctor orders it

Older adults accumulate silent, treatable problems — kidney function declining with age and medication, bone density falling after menopause or with low vitamin D, anaemia from a slow gastrointestinal bleed, an under-active thyroid mistaken for 'just getting older'. Screening the set together is also how drug doses get reviewed, since many common medicines are cleared by the kidney.

Symptoms that lead to this test

  • Increasing tiredness, breathlessness or unsteadiness
  • Unintended weight loss
  • Bone or back pain, loss of height, or a fall
  • Changes in urinary frequency or flow
  • Routine annual review, particularly on long-term medication

Understanding your results

Age changes how results are read rather than what is measured. A mildly reduced eGFR is common with age and is interpreted alongside the urine albumin and the trend, not in isolation. Anaemia in an older adult is never assumed to be dietary — it is investigated, because a slow gastrointestinal blood loss presents exactly this way. A raised PSA has many benign causes including an enlarged prostate, infection, recent catheterisation, cycling and ejaculation, so it is a prompt for a urology conversation rather than a conclusion.

Typical adult reference ranges

Indicative only. Reference ranges vary between laboratories and between testing methods, and shift with age, sex and pregnancy — always read your own result against the range printed on your own report, with the doctor who ordered it.

ParameterIndicative rangeUnit
eGFR90 or above (normal for age is interpreted individually)mL/min/1.73m²
PSA (total)Below 4 (commonly used, age-related)ng/mL
Haemoglobin13.0–17.0 (men) · 12.0–15.0 (women)g/dL
Serum calcium8.6–10.2mg/dL
Vitamin D (25-OH)30–100 (sufficient)ng/mL
TSH0.4–4.0mIU/L

Why the preparation matters

Ten to twelve hours of fasting for the glucose and lipid components — but do not stop or delay regular medication (blood pressure, heart, thyroid, anti-epileptics) to fast unless a doctor has said so, and take them with a little water. If you are on a diuretic, mention it: it changes sodium, potassium and uric acid, and the report is read differently as a result. Avoid vigorous exercise and ejaculation for 48 hours before a PSA sample, both of which can raise it.

Who should consider this test

  • Adults over 60, annually
  • Anyone on three or more long-term medications
  • People with known hypertension, diabetes or heart disease
  • Post-menopausal women, for the bone and vitamin D panel
  • Men over 50 discussing prostate screening with their doctor
