Preparation
10–12 hrs of fasting is required. Water is allowed. Morning collection is preferred for accurate sugar and lipid results.

What this test measures

Everything in the essential panel plus the parameters that turn a screening test into a risk assessment: free T3 and free T4 with anti-TPO antibodies (the autoimmune thyroid markers), a full iron study with ferritin, and bone chemistry — calcium, phosphorus, alkaline phosphatase and vitamin D — read together.

Why a doctor orders it

Clinicians order the advanced version when the basic panel is likely to be insufficient: an existing diagnosis being monitored, a strong family history, symptoms that a 70-parameter screen has already failed to explain, or simply an adult over 40 for whom the extra thyroid antibody and bone markers change what happens next.

Symptoms that lead to this test

  • Symptoms that a routine panel did not explain
  • Known thyroid, cholesterol or blood-sugar problems under follow-up
  • Persistent hair fall, brittle nails or unusual fatigue
  • Bone or joint aches, or a previous low-impact fracture
  • Planning a pregnancy, or a significant lifestyle change

Understanding your results

The additions mostly answer 'why', not 'whether'. A raised anti-TPO alongside a high TSH commonly points to an autoimmune cause for an under-active thyroid rather than an iodine or medication one — a distinction that changes monitoring, not a diagnosis you can make from the report. A low ferritin with a normal haemoglobin suggests iron stores are running down before anaemia appears. All of it needs clinical correlation.

Typical adult reference ranges

Indicative only. Reference ranges vary between laboratories and between testing methods, and shift with age, sex and pregnancy — always read your own result against the range printed on your own report, with the doctor who ordered it.

Why the preparation matters

Ten to twelve hours of fasting, for the same lipid and glucose reasons as the essential panel. Two extra points matter here: biotin supplements (common in hair and nail formulations) can interfere with several hormone immunoassays including thyroid tests, so mention them, and take thyroid medication AFTER the sample rather than before, so the result reflects your usual state rather than the peak after a dose.

Who should consider this test

  • Adults over 40, or over 35 with risk factors
  • Anyone already on thyroid, diabetes or cholesterol medication
  • People with a first-degree relative who has an autoimmune condition
  • Post-menopausal women, for the bone panel
  • Anyone whose previous checkup flagged a borderline value
