Preparation
10–12 hrs of fasting is required. Water is allowed. Morning collection is preferred for accurate sugar and lipid results.

What this test measures

Thirty-two parameters covering the standard lipid profile plus the markers that a standard profile misses: apolipoprotein B (Apo B, a count of the atherogenic particles rather than the cholesterol inside them), lipoprotein(a), high-sensitivity CRP for vascular inflammation, and homocysteine.

Why a doctor orders it

A meaningful minority of people who have heart attacks have an unremarkable LDL cholesterol. Apo B and Lp(a) describe risk that LDL alone does not — Lp(a) in particular is largely genetically determined, which is why it is usually measured once in a lifetime rather than tracked. hs-CRP adds the inflammatory dimension. The panel is ordered to refine risk, especially where family history and the basic lipid profile disagree.

Understanding your results

These markers describe probability, never certainty: a favourable panel does not exclude coronary disease and an unfavourable one is not a diagnosis. hs-CRP is a non-specific inflammatory marker and rises with any recent infection, injury or flare of arthritis, so a raised value is usually repeated at least two weeks after any illness before it is taken as a vascular signal. Lp(a) is treated as a lifelong risk modifier once measured. All of it belongs in a conversation with a cardiologist alongside your blood pressure, smoking status and family history.

Why the preparation matters

Ten to twelve hours of fasting keeps the triglyceride and calculated LDL values comparable with your previous reports. Homocysteine rises after a protein-rich meal, which is a second reason for the overnight fast. Avoid alcohol for 24 hours before the draw — it raises triglycerides sharply and briefly.
